Professional Medical Care
You can trust that the care provided in our facility is directed by a physician. Physicians make regular visits to our facility and are on call 24 hours a day to meet the needs of our residents. Our professional team provides the highest level of care available.
A comprehensive plan of care for each resident is implemented to ensure individual goals are achieved. We have registered nurses and certified nursing assistants as well as visits from psychiatrists, podiatrists, dentists and hospice.
Rehab Services
Our therapy staff is anxious to meet you, evaluate your needs, start you on your personalized rehabilitation program and make your stay here pleasant and productive. Our goal is to get you back to your prior level of function as quickly as possible.
We provide Physical Therapy, Occupational Therapy and Speech Therapy. Your physician will decide which of these you should receive. We strive to get you evaluated your first day unless you arrive later in the day. If that is the case, you will be seen the next day. (The only exception could be a late Friday or Saturday arrival.) After your evaluation, your therapists will develop an individualized treatment plan in conjunction with your physician.
What is Occupational Therapy?
Occupational Therapy (OT) helps people with disabilities to return to their lives through the use of therapeutic methods, applied and assistive technology and environmental modifications. Occupational therapy retrains patients in the activities of daily living such as bathing, dressing, grooming and feeding.
What is Physical Therapy?
Physical Therapy (PT) is for patients with pain and/or movement dysfunction resulting from disability or disease. Through proper evaluation and treatment, physical therapy can minimize pain and disability and maximize functional potential. Physical therapy adds to a patient’s strength, mobility and fitness.
What is Speech Therapy?
Speech Therapy (ST) offers comprehensive evaluation and treatment for individuals who have developed speech, swallowing or cognitive problems following an injury or illness.
Culture Change and Complete Dining Services
We strive to make each resident feel as though they are in their own home while at Grand River Health Care. Our staff honors resident choices and preferences by getting to know each resident and what their likes and dislikes are. We establish a fundamental relationship with each individual person to ensure that the needs and wants of that person are met.
Our home has begun a culture change journey by respecting the choices that our residents make. For example, each resident has the opportunity to make their own daily schedule by choosing what time they will eat, when they bathe, what time to wake up each morning and what activities they want to do during the day. Our facility also has a complete dining service in which the resident can select what they want to eat each day. We offer a variety of nutritious meals and snacks that residents can choose from based on their individual preferences.
Behavioral Health Services
Difficult behaviors due to mental illnesses sometimes lead to isolation and over medication. Grand River Health Care welcomes these behaviors in an environment that is more accepting and tolerant. Where behaviors are managed and redirected without over sedation. Specially trained staff aim to maximize independence, quality of life and are sympathetic to residents’ needs and their loved ones who are faced with this challenge. Grand River Health Care has been successful in restoring harmony within social and family relationships.
The Horizons program is designed for the geriatric resident who exhibits behaviors which preclude them from placement in a typical long-term care setting. Horizons meets the needs of the older person finding new meaning for quality of life, in light of issues such as grief and loss, isolation and declining health.
